Adaptive Transition Care™ is a private, nonclinical advisory and support practice for individuals and families navigating profound life transition.
It is designed to help people preserve dignity, emotional steadiness, meaningful structure, contribution, relationships, wellness, and future possibility during seasons when life no longer functions as it once did.
Adaptive Transition Care™ supports the whole person, not only the diagnosis, age, care need, or life disruption. It helps individuals and families ask a deeper question:
How do we continue building a meaningful life in the midst of change?
Who we serve & How we support
Adaptive Transition Care™ is for individuals, families, and care partners navigating significant life changes. These transitions may include aging, neurological change, chronic illness, sensory change, caregiving responsibilities, retirement, grief, burnout, identity change, or a major shift in daily functioning. It is designed for moments when life is changing in ways that require more than ordinary support, and when a person or family needs guidance, steadiness, and a more thoughtful way to move forward.
This work is especially suited for intellectually engaged, spiritually grounded, creative, professional, or purpose driven individuals who need more than task support alone. Many people in transition are still deeply alive with wisdom, memory, creativity, leadership, faith, curiosity, and the desire to contribute. Adaptive Transition Care™ helps protect and nurture those parts of life, even as daily rhythms, capacities, roles, and responsibilities may be changing.
Adaptive Transition Care™ may support older adults entering a new season of life, families caring for a loved one through transition, adult children coordinating care and support, and retired professionals, clergy, artists, educators, civic leaders, and creatives seeking continuity of purpose. It may also support individuals who want to remain connected to meaning, contribution, beauty, relationship, and possibility during a time of profound change.
Adaptive Transition Care™ is not therapy, medical care, nursing, home health care, legal advising, or financial planning. It is a whole person advisory and accompaniment practice that works alongside existing care systems to support human continuity, dignity, and adaptive flourishing.
This work helps individuals and families move from overwhelm toward steadiness, from fragmentation toward clarity, and from uncertainty toward a more supported way of living. Through conversation, assessment, planning, practical organization, embodied practices, family support, and meaningful life design, Adaptive Transition Care™ helps people discern the next faithful shape of life.
The goal is not to pretend that nothing has changed. The goal is to help meaningful life continue through change.
The Six Domains of Adaptive Transition Care™
Adaptive Transition Care™ supports six interconnected domains of life.
1. Emotional Steadiness and Well Being
Supporting grounding, reflection, emotional pacing, self trust, nervous system steadiness, and the ability to move through change with greater spaciousness.
2. Organization and Adaptive Functioning
Supporting routines, planning, executive functioning, communication, follow through, household systems, technology needs, paperwork, and daily life structure.
3. Meaningful Life and Contribution
Supporting purpose, creativity, intellectual engagement, leadership continuity, mentoring, legacy work, memoir, philanthropy, spiritual calling, and meaningful participation in life.
4. Adaptive Wellness and Embodied Practices
Supporting restorative practices, mindfulness, gentle movement, breath, meditation, body awareness, rest, pacing, and adaptive wellness routines.
5. Relationships and Support Ecosystem
Supporting family communication, caregiver alignment, role clarity, support mapping, shared language, and healthier systems of care.
6. Adaptive Possibility and Future Vision
Supporting future planning, imagination, adaptive goals, quality of life, meaningful experiences, and the ability to see possibility without denying reality.
Levels of Service
Adaptive Transition Care™ is offered through three levels of support.
Essential Care
Foundational steady support for individuals and families who need thoughtful guidance, reflective conversation, light organization, and ongoing adaptive structure.
Essential Care is often appropriate when a person is navigating change with relative stability but would benefit from consistent support, clarity, and companionship.
Typical structure: Approximately 2 direct service hours per week or 8 hours per month.
Integrative Care
Expanded continuity support for individuals and families navigating layered transition, shifting routines, family communication needs, meaningful life questions, and active planning.
Integrative Care may include deeper support around executive functioning, family ecosystem alignment, adaptive wellness, purpose, care rhythm, and 90 day planning.
Typical structure: Approximately 4 direct service hours per week.
Immersive Care
Highly personalized, concentrated support for individuals and families navigating significant transition, deep restructuring, legacy work, intensive planning, or a season requiring greater presence and continuity.
Immersive Care may include longer visits, family meetings, transition intensives, legacy support, restorative practices, and more involved adaptive planning.
Typical structure: Approximately 6 direct service hours per week, usually offered in 2 hour or 4 hour blocks.
The Process
Adaptive Transition Care™ begins with a Discovery Conversation.
This conversation helps identify what is changing, what is working, what feels unclear, what support is already in place, and what meaningful life looks like now.
From there, an Adaptive Transition Assessment may be completed across the six domains of care. A 90 Day Adaptive Transition Plan may then be created to identify the recommended level of support, adaptive priorities, desired outcomes, and next steps.
The process is dignifying, strength forward, relational, and practical.
